Timeline to report?

Workplace painted a hallway with aircraft paint. Was told they were going to do investigation but come to find out, nothing was done. It’s been over a year I think, and just found out. What is the time limit to report such instances.

I've been tasked with updating the company safety manual. It's a stapled pamphlet 17 pages long. Here's my favorite part so far. PS does anyone have an encapsulating file for Earthwork construction companies?
