-Chemical Examination of CSF:-

1)-Glucose
-normal concentration of glucose in CSF samples is 45-80 mg/dL or 60-80% of that in the plasma
يعني نسبته في CSF بتكون حوالي تلتين نسبته في بلازما الدم.

-Absolute decreased CSF glucose level
اي نقص في مستوي الجلوكوز في عينة CSF دا بيكون دلالة علي
bacterial or fungal meningitis

-normal CSF glucose concentration but with increased number of WBC⏩viral meningitis
اما اذا كانت النسبة طبيعية مع زيادة في عدد كريات الدم البيضاء هنا بنشك في وجود التهاب السحايا نتيجة عدوى فيروسية

2)-Lactate

-Conditions associated with elevated CSF lactate
ترتبط مع
decreased blood flow or hypoxia
نقص تدفق الدم او نقص الاكسجين
(eg, head trauma), such as the following:-
-Intracranial hemorrhage
-Cerebral arteriosclerosis
-Hypotension
-Metastatic cancer
-Trauma
-Seizures
-Bacterial meningitis
-Mycoplasma meningitis

-تقيم تركيز حامض اللاكتيك في سائل CSF بيكون مفيد في
diagnosis and management of different types of meningitis
على سبيل المثال
1)-CSF lactate >35 mg/dL is seen in patients with
?(bacterial meningitis)

CSF lactate 25-35 mg/dL is seen in patients with
?(tubercular and fungal meningitis)

CSF lactate < 25 mg/dL is seen in patients with
?(viral meningitis)

ملحوظة مهمة:-
تحتوي ال RBCs على تراكيز عالية من
lactate and LDH
لذلك تؤدي الى
xanthochromic CSF samples with elevated hemoglobin and/or RBC
وف النهاية تؤدي الى نتائج مرتفعة خاطئة
falsely elevated lactate and LDH results

-يشمل ارتفاع (CSF LDH)
كلاً من
-Intracranial hemorrhage
-Bacterial meningitis

-بينما يشمل ارتفاع (CSF glutamine)
في حالات
-liver disease,
-inherited urea cycle disorders
-Reye syndrome
-حيث ان النسبة الطبيعية لل glutamine
normal glutamine concentration in CSF is 8-18 mg/dL

?IgG index
-مؤشر IgG هنا نطبق معادلة
IgG index = [IgG CSFAlbumin blood]/[Blood IgG Albumin CSF]×100

حيث ان IgG index بيدينا فكره عن دخول IgG molecules الى داخل CSF عند حدوث
damaged blood-brain barrier

-القيمة الطبيعيه تكون
normal value of IgG index (generally 0.25-0.7)
حيث لو كانت
generally if IgG index is greater than 0.7
دا دليل على
actively producing IgG within CSF, while the blood-brain barrier is intact
بينما لو كانت
decreased IgG index reflects damaged blood-brain barrier
مثل
(stroke-tumors-some meningitis)
