إذا بدأ الشكُّ والتفسير للأفعال الصغيرة من الطرفين يملأ الحيّز بين الفعل ورد الفعل… وإذا غابت اللهفة للقاء والسؤال والتفقّد…أوقِف هذه العلاقة لمدة من الزمن ولربما إلى الأبد …حُبا لا بُغضاً…فبهذا إما تقلل الخسائر أو تمنعها..

واهتم أنت بسريرتك وداخلك وابدأ بتنقيته تجاهه…قد تنجح وقد تفشل..وقد ينجح هو وقد يفشل…

فإن الرجل ليَكونن ذكرى جميلة …أفضل عندي من أن يكون حاضرا مريرا ينتهي إلى مالا يُحمد عقباه…

الحروب والكروب وميَلانِ الدروب تعصف بالعلاقات عصفا..فلا يثبت منها إلا الراسخة المُتأصلة إلى أعماق بعيدة…

This summary will make management of infective endocarditis fun

🫀Infective Endocarditis🫀**

👉 Risk(IVDU, prosthetic valve, indwelling IV catheters, HD)
➕
👉(Fever and/or Changing murmer) with any sign of metastatic infection

💡Any patient who has infection in 2 different sites ➡️think IE.

e.g evidence of discitis+stroke (back pain and altered mental status

💡DUKE’S Criteria are useless in acute setting.

⬇️⬇️

👉Send 2-3 blood cultures (ideally from different peripheral sites)
👉Grab the US and check the heart (any valvular regurgitation, vegetation) mostly patient will need TEE later on.
👉Send (CBC, urinalysis, RFT and electrolyte )

➕➕

🛑 Low threshold for detailed imaging as follows:-

👉 patient with dyspnea and desaturation➡️ CXR and CT chest to rule out right sided IE (multiple cavitating lesions)

👉Confused patient➡️initial CT brain with MRI➡️ to rule out cerebral infarction and mycotic aneurysms

👉 severe back pain➡️ MRI spine to rule out discitis caused by staph

👉Joint pain➡️ scan the joint.

👉Splenic ultrasound ➡️splenic infarction

🛑Start antibiotics after blood cultures 🔜

👉 Acute, native valve: Vancomycin
and Cefazolin (MSSA and MRSA) mostly no need to cover gram negative

👉Subacute, native valve: Vancomycin and Ceftriaxone (strept. Viridans, MRSA, gram negative bacilli)

👉Prosthetic valve, acute onset, <1 year post surgery: Vancomycin and Gentamicin plus cefepime (MRSA, gram negative bacilli)

👉Prosthetic valve, acute onset, >1 year post surgery: Vancomycin and Gentamicin plus Cefazolin (MRSA, MSSA, gram negative bacilli)

💡 Cefazolin has poor CNS penetration➡️ switch to Nafcillin if confused
💡Refampin will be added after 5 days, do not start immediately (risk of resistance)

💡Consult cardiothoracic surgery if:-

👉Heart failure
👉Valvular regurgitation
👉Persistent embolic phenomena despite treatment

💡Any heart block on monitor or ECG➡️ urgent echo to rule out valvular abscess (consult surgery)
💡Consider removal of any hardware especially if repeat culture doesn’t show clearance of infection.
💡Involve neurology for strokes (no anticoagulation)
💡Involve infectious disease for detailed assessment
