Very Practical point?
When you have raised TSH, with a normal T4, Please classify your patient accordingly:

1- patient with hypothyroid but on medication
2- undiagnosed without symptoms
3- undiagnosed with symptoms

So:

1- This gives you two things: A) either insufficient dose of thyroxin, or B) **non compliance

2- This needs to berepeated and followed because it gives a clue of subclinical hypothyroidism**, unless TSH is exceeded 10 for two times.

3- This is hypothyroidism, needs to start thyroxin.
