🛑Symptomatic hypoglycemia from intentional sulfonylurea overdose is treated with both dextrose and octreotide.

✅In adults, the dose of octreotide is 50 to 100 mcg administered by intramuscular, or subcutaneous, injection every six hours.

✅In children, the dose of octreotide is 1 to 1.5 mcg/kg (up to 50 mcg) every six hours.

✅the role of octreotide in hypoglycemia associated with therapeutic use of sulfonylureas is unclear.

🛑استخدامات دواء colchicine في مرضى القلب كالاتي
✅لدواء colchicine استخدامات عديده في مرضى القلب نذكرها باختصار

✅اولا في حاله التهاب التأمور pericardial disease
✅في حاله acute and recurrent pericarditis يوصى الجايدلاين الاوروبي ESC باستخدام دواء colchicine خيار اول بينما الجايدلاين الأمريكي يوصى باستخدام دواء colchicine في حاله كان pericarditis بعد MI
✅الجرعه في هذه الحالتين حسب الوزن لو كان وزن المريض اقل من 70 كيلو ياخذ 0.5mg مره في اليوم لو وزنه اكثر من 70 ياخذ نفس الجرعه ولكن مرتين في اليوم لمده ثلاثه أشهر
✅ممكن في حاله
🛑Post-Pericardiotomy Syndrome pericarditis
🛑Dressler's syndrome pericarditis
لمده شهر بنفس الجرعه المذكوره

🛑ثانيا في حاله ACS او chronic coronary syndrome
✅متى بضبط؟
ذكرته في منشور سابق ؛لمنع Secondary attack في حاله ضبطنا جميع risk factors ل ACs مثل dyslipidemia و hypertension و DM ..الخ
هنا ممكن نعطي 0.5mg في اليوم قد نستمر به مدى الحياة

🛑اخيرا هناك استخدام غير مشهور ولكن قد نستخدمه لمنع atrial fibrillation بعد العمليات الجراحية او Post-ablation atrial fibrillation بجرعه 0.5mg مره إلى مرتين في اليوم لمده شهر إلى شهرين

وهل هناك غيرهن ؟

اولاً ليش هذه فقط
السبب يعود انه تمت عدة دراسات ع الأدوية التي تقلل من الأمراضيه والوفيات فكانت هذه هي
بينما البقية وجدوا أنها ي أما إنها لا تقلل او أنها تعمل ع زيادة الأمر سوئاً

أما بالنسبة ل متى أختار كل واحد وأيهنا أفضل  يعتمد ع المريض والحالة

بالنسبة الأفضلية كانت لل Carvedilol
ع شان نعرف السبب دعونا أولاً نعلم كيف يشتغل
الدواء هذا يعتبر
carvedilol is a third-generation nonselective β-blocker that competitively blocks β1-, β2-, and α-receptors with some antioxidant properties.
وطبيعة الحال
Approximately 80% of adrenergic receptors in the normal myocardium are β1-receptors.
However, due to β1 downregulation in HF, β2 may rise up to 40% of the total adrenergic receptors.
لذالك هذا ميزة لدواء 
وأيضاً بما أن ال
carvedilol blockade of α1-receptors causes vasodilation of blood vessels and might help to improve afterload
لكن هذا قد يكون
higher risk for hypotension.
أيضآ من جانب آخر وجد في دراسه أن ال carvedilol أكثر واحد في تقليل الوفيات

طيب هذا الكلام لا يعني أن البقية ليس لهم منفعة
بالنسبه لدوائين الآخرين يعتبرين

والذي تعمل
β1-receptors to a much greater extent than β2-receptors,
without antioxidant properties or vasodilating effects.
وقد كان ال
Metoprolol is 75-fold selective, whereas bisoprolol has approximately 120-fold
ف هذه نفضلها في حالة انه هناك مانع لاستخدام الدواء الأول مثل الربو وأمراض السكر
او في حالة ال HR أكثر من ال BP

هل هناك دواء اخر ؟
نعم هناك دواء اسمه
Nebivolol
ويعتبر
  selective β1-receptor antagonist
ولة خاصية
vasodilatory
والسبب يعود
  NO production via activation of β3-adrenergic receptors.
وهذا هو أكثر دواء انتقائي ع  β1

It is approximately 3.5 times more β1-selective than bisoprolol
لكن وجد انه لا يقلل من الوفيات والأمراضية إلا في كبار السن الذي أعمارهم  70 سنة او أكثر
لكن غير مصرح من ال FDA فقط من الجانب الأوربي.
