A 62 years old male Libyan patient soldier residing in Benghazi.

◽️Admitted on 25th October
complain with ➡️ Dyspnea from 4 days relived by Nebulizer.

◽️presented with ➡️ orthopnea, PND, sweating, Dry cough sometimes become productive with white sputum & wheezy chest.

◽️Suffered from ➡️ L.L swelling relieved by Diuretics, abdominal distention & intermittent claudication

-H/O Heart burn relieved by PPIs & Dysphasia
-No H/O loss of wt

PMH ➡️ He was hospitalized 4 times due to the same illness.
H/O Heart failure

PSH ➡️ There's no H/O Surgery
No H/O of blood transfusion

-No family history of the same illness or any other chronic DS.

-Heavy smoker since 46 years.

-There's no H/O alcohol intake
No H/O drug abuse
No H/O animal contact
