Dylan was coughing and drawing blood into his lungs due to the gunshot wound he received. His body involuntarily performs reflex movements such as stinging as an automatic response to drain the blood from the airways of its members. In this place, the situation of people being aware or hiding at the level of pain/suffering is different.

I still find it a little funny that Dylan got that sticker on his car because he loves DJ Spooky; it's pretty obvious that those couples have the Twin Towers and bombs fell on them (I mean, he said the Twin Towers were hit on Dylan's 20th birthday, AFTER Eric said he missed the plane and ran to New York.)
